This is the checklist printed on page 2 of Uttar Pradesh's own application form, and it is the same on the rural and the urban form. The form also makes compulsory the head's Aadhaar (or enrolment) number, the head's voter ID number, bank account with IFSC, the household's total annual income, gas connection details, and an Aadhaar number for every member.

Who cannot get a card in Uttar Pradesh
A household is not eligible if anyone in it pays income tax, or it owns a four-wheeler, an air conditioner or a generator of 5 KVA or more — in rural areas a tractor or harvester also counts. In towns, a residential plot or flat above 100 sq m, or commercial space of 80 sq m or more, rules a household out. In villages the test is land: more than 5 acres of irrigated land, though the limit is 7.5 acres in Bundelkhand and in Sonbhadra district south of the Kaimur range. The income ceiling is ₹3 lakh a year in towns and ₹2 lakh in villages.
What you get, and what it costs
How much grain you get depends on the card, not the state. An Antyodaya (AAY) household gets 35 kg a month for the whole household. A Priority Household (PHH) gets 5 kg per person per month, so the total depends on how many members are on the card.
It costs nothing. Foodgrains under the National Food Security Act have been free since 1 January 2023, and the free period was extended for five years from 1 January 2024.

The ₹3 a kg for rice, ₹2 for wheat and ₹1 for coarse grains quoted on most websites are Schedule I prices, which the Act guaranteed only for three years from its commencement in 2013.
Whose name is the ration card issued in?
The eldest woman of the household who is 18 or older is the head of the household for the purpose of issuing the card. If the household has no woman aged 18 or above, the eldest male member is the head — and when a female member turns 18, she becomes the head in his place.
